Transition Services

GordyThe Day Transition program is made up of two separate programs; the Day Services Program and the Transitional Living Program. Clients in all three programs check in at the Transition Services office where they have access to case management, life skills training, employment skills training, and individualized treatment. Clients have access to a resource center where they can use the computers and community resource materials to complete assignments, create and update resumes and cover letters, research housing opportunities, explore recovery options, etc. Clients also access the computers to register with the Oregon Employment Office and to track job listings.

Because many of the Day Transition clients live on their own, the program is able to offer more community-based, practical assistance than other areas of the agency. Staff is able to help clients work through real-life problems as they occur as opposed to discussing hypothetical situations.

Day Services Program

Clients in the Day Services Program check in at the Day Transition office daily while living in community placements outside of Youth Progress Association. Clients in the Day Services Program live independently, with family, or in foster homes that are not contracted with YPA.

Transitional Living Program

Clients in the Transitional Living Program check in at the Day Transition office two or three times each week while living in apartments operated by Youth Progress Association. This program allows clients to experience independent living while maintaining the safety net of the program. Many clients in the apartments transitioned there from other areas of the agency.
